Why this practice was nervous about switching

If you run an allergy practice, this will sound familiar. This group was moving to a new practice management system in early 2025 and needed a new billing team at the same time. That is the moment most practices see claims stall and cash fall for months. They also had:

  • Billing that is easy to get wrong. Allergy testing is billed by the number of tests, serum by the dose, and shots every week. Same-day office visits need the right modifier or they get denied.
  • A lot of money owed by patients. High deductibles meant patients paid about a quarter of everything collected. Those balances age fast if nobody follows up.
  • No room for a slow start. About 24,500 claims a year across 40 insurance plans, and about $198,000 a month that payroll and rent depended on.

What we did, and why it matters to you

  • Set up every insurance plan and fee schedule before go-live. Claims went out clean on day one instead of piling up while the new system was fixed.
  • Checked allergy codes before claims were sent. Testing units, serum doses and injection codes were matched to each plan's rules, so they were paid the first time.
  • Showed every dollar, including the bad news. Every payment and adjustment was posted with its reason, so denials were never hidden as contract discounts.
  • Finished every denial. Each one was fixed and resent, appealed, or closed with a written reason. Nothing was left to age out.
  • Made it easy for patients to pay. Clear statements and card payments meant patient balances were paid instead of left to age.

What that means in dollars

For a practice collecting about $2.4 million a year, every 1% of collection rate is worth roughly $25,000 a year. Moving from 92% to 97.6% at this size would mean about $135,000 more a year, for care the practice has already delivered.

How we measure: net collection rate by date of service, using claims at least 120 days old. Self-pay visits excluded. Refunds subtracted from payments. Written off means money lost to denials and other non-contract adjustments, as a share of what the practice was allowed to collect. We use the same method for every client.

Where we are still improving

No billing year is perfect, and we tell clients the truth about their numbers. Early 2026 dipped slightly to 96.5% when deductibles reset and patients owed more of each bill. When issues come up, we find the cause, fix it, and tell the practice exactly what happened. Next, we are checking benefits and authorizations before visits, especially each January.
